Dr.Jacob prides himself on providing friendly, personal care. He will treat you as if he were treating a member of his own family. Treatment is provided for one person at a time and we do not double book because we feel you deserve our undivided attention.
Your initial visit will include a thorough oral exam, X-rays, and a personal discussion with Dr. Jacob regarding your treatment options. Please be sure to fill out the enclosed Health History and Patient Registration forms before your appointment so we can spend as much time as possible addressing your dental concerns.
If you have X-rays or records from a previous dentist, please make sure to fill out the Dental Records Release Form and send it to your previous dentist.
As a courtesy to you, we will be happy to research your dental plan benefits so you can have an idea of what benefits your plan may contribute toward your treatment. Please be sure to have your insurance information available when you come in.
